Guidelines for Completing the Library Faculty Evaluation Form

1.        Each applicant for promotion will be evaluated by his/her Peer Evaluation Committee using the Library Faculty Evaluation Form.  Each applicant will be evaluated in the areas of Job Performance, Professional Activities, and Demonstrated Characteristics.  In each area the applicant will be rated on the following basis:

OUTSTANDING                    = 4.00-3.01 points

VERY GOOD                         = 3.00-2.01 points

SATISFACTORY                   = 2.00-1.01 points

UNSATISFACTORY             = 1.00-0.01 points

Decimal values may be used, if desired.  A Peer Evaluation Committee must provide a rating in each area.

2.        If a Peer Evaluation Committee would like to emphasize any particular item(s) within an area, this may be done by placing a + (favorable) or an X (unfavorable) by the specific item(s).  The committee may comment concerning the applicant in each area.

3.        In addition to providing a rating in each area on the form the Committee may add at the end of the form a list of the applicant's strengths and weaknesses.  A Committee which gives Outstanding (4.00-3.01) or Unsatisfactory (1.00-0.01) ratings must justify these ratings with appropriate statements concerning the strengths or weaknesses of the candidate.

4.        To be recommended for promotion, an applicant must receive a score in each area no lower than that indicated as the minimum for the rank for which the applicant is being considered.

For promotion to Professor, the minimum score is 3.00.

For promotion to Associate Professor, the minimum score is 2.70.

For promotion to Assistant Professor, the minimum score is 2.50.
