Texas Award for Performance Excellence (TAPE) Category 3 Customer and Stakeholders Internal Survey
Purpose: The purpose of this survey is to gather information about our internal processes with St. Philip’s College customers and stakeholders.
Procedures: If you agree to participate in this research; we ask that you complete the attached survey. The survey should take only 3 to 5 minutes of your time.
Risks and Benefits: The information you share will be used to assist in the development of our Texas Award for Performance Excellence (TAPE) application. You may refuse to answer any questions on the form. We can provide you information about programs and services that you can contact for help if you need it. We will not share your personal information with anyone, including staff, faculty and students. There are no direct benefits of participating in this survey.
Confidentiality: The records of this study will be kept private. Anything you tell us will remain confidential. In addition, we will not include any information that will make it possible to identify you. Your email will be kept with this survey for clarification purposes only. The surveys will be stored electronically in a secure fashion. Any hardcopies of the surveys developed will be stored with the Coordinators of the Texas Award for Performance Excellence (TAPE) project.
Voluntary nature of study: Your decision whether or not to participate in this research will not affect your current or future relations with St. Philip’s College.
